Did You Know? Di/Di Twins Can Be Fraternal OR Identical!

First things first…What Does Di/Di Mean?

Di/Di is short for dichorionic diamniotic.

In simple terms:

👶 Baby A has their own amniotic sac.
👶 Baby B has their own amniotic sac.
🏠 Each baby also has their own chorion.

The two placentas may look completely separate, or they can grow next to each other and appear fused. This is one reason ultrasound technologists look at more than simply the number of visible placental masses when determining chorionicity.

When you hear “Di/Di twins,” you might automatically think fraternal twins.

But here’s a fun twin fact that surprises a lot of parents:

Di/Di twins can actually be fraternal OR identical!

Let’s break down what all those “Di’s” actually mean.

When Di/Di Twins Are Fraternal

Most Di/Di twins are dizygotic, better known as fraternal twins.

This happens when:

🥚 Two separate eggs are released

✨ Each egg is fertilized by a different sperm

This equals:
👶👶 Two genetically different babies

Dizygotic twins develop with separate chorions and separate amniotic sacs, making them Di/Di.

They can be:

💙 Boy + Boy
💗 Girl + Girl
💙💗 Boy + Girl

They are genetically similar in the same way other siblings are — they just happen to share a birthday!

But Di/Di Twins Can Also Be IDENTICAL!

Here’s where it gets interesting.

Identical, or monozygotic, twins begin with:

🥚 ONE fertilized egg

That single embryo then divides into two embryos.

If that division occurs very early in development, each baby can develop their own chorion and their own amniotic sac.

The result? Identical Di/Di twins!

Medical literature estimates that roughly 30% of monozygotic twin pregnancies can be dichorionic diamniotic, while later splitting generally results in other arrangements, such as monochorionic diamniotic twins or monochorionic monoamniotic

So If They're Both Boys or Both Girls, How Do You Know?

This is another fun part!

If Di/Di twins are one boy and one girl, they are dizygotic/fraternal in the usual clinical setting.

But if they're...

💙 Boy + Boy
or
💗 Girl + Girl

...simply seeing two sacs and two chorions doesn't prove whether they're identical or fraternal.

Research confirms that monozygotic twins can be either monochorionic or dichorionic, while dizygotic twins are characteristically dichorionic.

If parents want to know with certainty whether same-sex Di/Di twins are identical, DNA testing can determine zygosity.

Why Does Your Ultrasound Provider Care About Di/Di vs. Mo/Di?

During a twin pregnancy, one of the important things evaluated on ultrasound is chorionicity and amnionicity — essentially, how the babies' placentas and sacs are arranged.

This isn't just a fun twin fact. It helps the medical team determine how the pregnancy should be monitored.

The International Society of Ultrasound in Obstetrics and Gynecology (ISUOG) emphasizes determining chorionicity early in pregnancy because different types of twin pregnancies have different risks and surveillance recommendations. Chorionicity is most accurately assessed early in pregnancy.
